Troponin T test for one-hour diagnosis of heart attack

Results from the TRAPID-AMI clinical study confirm a novel approach for a more rapid diagnosis of heart attack in patients with acute chest pain. The strategy is based on the cardiac troponin T high-sensitivity test (cTnT-hs) and reduces the observation time needed to rule-in or rule-out a heart attack from 3-6 hours to just 1 hour. It is well established that a fast and reliable diagnosis of heart attack is critical because every hour of delay from the onset of symptoms to treatment increases the mortality risk. Troponin is a heart muscle protein that is released into the blood stream during a heart attack. A limitation of the earlier generations of blood tests was the time required to detect the troponin release, sometimes requiring up to six hours with less sensitive troponin tests. The mortality rate of heart attacks is highest within hours of onset, so an early diagnosis and initiation of treatment greatly impacts outcome and potentially saves lives.
The Elecsys cardiac Troponin T high-sensitivity (cTnT-hs) test from Roche detects cardiac troponin which is the preferred biomarker for the diagnosis of heart attack in clinical practice. In combination with an electrocardiogram (ECG), it has become the gold standard for the diagnosis of heart attack. The high sensitivity of the Roche cTnT-hs assay in conjunction with this novel procedure significantly accelerates "rule-in" and "rule-out" decision-making, thereby maximizing the potential for effective treatment. At the same time, the faster decision-making may help to better manage the emergency room workload and related costs for healthcare systems.

The recently launched new generation of automated breast ultrasound, Invenia ABUS, shifts traditional ultrasound from hardware- to software-based processing which results in excellent image quality, reproducibility and easy operation. The ABUS also includes intelligent imaging algorithms to help produce better image quality for easier identification of lumps, such as the tissue equalization algorithm, nipple shadow compensation, breast border detection and chest wall detection. Invenia ABUS’s Reverse Curve Transducer is designed to shape a woman’s anatomy and enhances patient comfort during the exam as well as breast coverage. The 15 cm wide filed-of-view, high-frequency transducer automatically creates uniform compression across the entire breast for enhanced anatomical detail and consistent image quality independent of the operator. Oncology workflow with advanced toolset for PET/CT

Carestream now offers an oncology reading workflow for PET/CT studies that equips clinicians with new native tools for its enterprise image management system that deliver both quantitative and qualitative information to help improve and expedite the reading process. The latest upgrade for the company’s enterprise image management platform incorporates advanced tools to aid in tumour detection and tracking. This can help radiologists deliver more meaningful reports while also enhancing collaboration with clinicians and patients. Traditional oncology features cannot adequately address the increased demand to be expected for long-term monitoring of oncology patients as the world’s population ages.
The new CT Perfusion and Subtraction modules contain interactive tools to ease the process of analysing and comparing three-dimensional (3D), CT perfusion (CTP) and digital subtraction angiography (DSA) images. This equips users to analyse CT brain perfusion scans for tissue perfusion and tissue blood volume, while X-ray angiography (XA) subtraction capabilities provide a DSA workflow for analysis of vessel blockages in areas with bony or dense soft tissue. Carestream also developed PET segmentation that allows users to examine tumour standardized uptake values (SUV) traces on the PET image without the need for a physical trace on a CT exam. This capability can play an important role in detection of early stage cancers. The company’s streamlined oncology reading workflow includes: automatic fusion of PET, SPECT, CT and MR images; automatic registration between volumetric datasets that provide efficient comparison of multiple time points; synchronized manipulations, colour maps, measurement tools and pre-defined layouts that aid in analysis; reproducible 2D and volumetric lesion segmentation and automated comparison to observe progression over time; threshold segmentation, spheroid region of interest (ROI), metabolic tumour volume (MTV) and total lesion glycolysis (TLG); bookmarks to catalogue lesions and speed future access and precise follow-up data for enhanced patient care. An efficient reading workflow is supported by use of Carestream’s native Vue Reporting module that can present interactive hyperlinks to critical images and automatic inclusion of quantitative analysis in the form of easy-to-understand comparison tables and charts, which oncologists and referring physicians prefer. Vue Reporting also offers bookmarks and lesion management reporting in accordance with international standards such as response evaluation criteria in solid tumours (RECIST). Reports and images can be shared using Carestream’s Vue Motion universal viewer that supports viewing on FDA-approved mobile devices.

Ultrasound application for treatment of structural heart disease

TrueFusion, an application on the new Release 5.0 of the ACUSON SC2000 cardiovascular ultrasound system, integrates ultrasound and angiography images. These integrated images can be used to guide cardiac teams when administering treatment for structural heart disease. Treatment options for structural heart disease are evolving rapidly. Increasingly, these minimally invasive procedures involve multimodality imaging and multidisciplinary clinical teams. To reliably diagnose structural heart disease and perform these complex interventions, clinical teams need detailed, real-time imaging information – specifically, real-time soft tissue and blood flow information from echocardiography as well as 2D imaging information from fluoroscopy – to be visible in one view for common orientation.  Addressing the need for fused images, the new TrueFusion application sends anatomical and functional markers as well as valve models from the ACUSON SC2000’s True Volume transesophageal echocardiography (TEE) transducer to an Artis with PURE angiography system, overlaying ultrasound information with live fluoroscopy images to navigate structural heart procedures. By directly and seamlessly integrating co-registration of Artis fluoro and ACUSON SC2000 echo into the workflow via machine learning-based probe detection and automated registration updates, TrueFusion enables clinical teams to identify soft tissue-based structures  that are provided directly from the integrated ultrasound system.  Not only can echocardiographers and interventionists better communicate and achieve more intuitive anatomical orientation during challenging procedures, but clinical teams potentially can reduce contrast usage and procedure time as well as patient and clinician X-ray exposure.  For the high innovation pace of novel procedures and devices in structural heart disease, data and image integration is now available for clinical decision support and real-time therapy guidance.

Compact mechanical ventilator

The integrated high-performance turbine enables the ventilator to be completely independent of compressed air. This makes it an ideal companion for all patient groups in the intensive care unit, emergency ward, recovery room or intermediate care, long-term acute care facilities, and during intrahospital transport. The HAMILTON-C1 offers the option of an integrated high flow oxygen therapy mode. With this enhancement, it provides a variety of ventilation and therapy options in one device, including invasive and noninvasive ventilation, and high flow oxygen therapy. In just a few steps, it is possible to change the interface and use the same device and breathing circuit to accommodate the patient’s needs. In pressure-controlled modes (PCV+, SPONT, PSIMV+), an optional feature enables the use of conventional speaking valves with the ventilator. Monitoring, triggering, and alarm management have been adjusted to allow their use.
