The Thalassaemia International Federation (TIF) has published the fifth edition of its guidelines for managing transfusion-dependent β-thalassaemia (TDT), incorporating the latest scientific developments and clinical practices. The updated resource, launched at the 50th Arab Health Medical Expo in Dubai, provides healthcare professionals with evidence-based recommendations for treating this severe genetic haematological condition.

The new edition represents a significant update to the federation’s long-standing clinical guidance, first published in 1999. It encompasses crucial aspects of thalassaemia management, including blood transfusion protocols, iron chelation strategies, and comprehensive approaches to medical complications. The guidelines have been expanded to address fertility considerations, nutritional requirements, quality of life measures, and psychological support for patients.

International expertise and collaboration

Fifty-six authors from leading medical institutions across multiple countries contributed to the guidelines, bringing together expertise from the United States, Italy, United Kingdom, Greece, Canada, Lebanon, and Thailand. The editorial team comprises prominent figures in thalassaemia research and treatment, including Professors Ali T. Taher, John B. Porter, Maria D. Cappellini, Khaled Musallam, and Dimitrios Farmakis.

Clinical implementation and accessibility

The guidelines feature detailed, step-by-step recommendations designed to support treating physicians in their daily practice. To enhance usability, the publication includes numerous summary tables that facilitate quick reference and improved comprehension of complex protocols. This practical approach aims to standardise care delivery across different healthcare settings.

Supporting multidisciplinary care

Dr Androulla Eleftheriou, Executive Director of TIF, emphasised the guidelines’ role in supporting healthcare providers: “The newly revised guidelines underscore TIF’s commitment to enhancing patient care and keeping healthcare providers well-informed with the latest research and clinical practices. We extend our deepest thanks to all the contributors of this publication and are confident that they will prove to be an invaluable tool for clinicians in their everyday practice, facilitating the provision of the most effective and up-to-date care possible.”

Global recognition and implementation

The guidelines have achieved widespread recognition since their initial release, with adoption by major scientific organisations including the European Haematology Association (EHA). This acceptance reflects their crucial role in establishing optimal clinical practices and advancing diagnostic and therapeutic interventions in thalassaemia care.

Continuing education and resource development

As part of its ongoing commitment to advancing thalassaemia education and knowledge, TIF maintains its programme of developing and distributing free guidelines covering various haemoglobinopathies. This includes resources for α-thalassaemia and non-transfusion-dependent thalassaemia (NTDT), supporting the medical community’s multidisciplinary efforts in patient care.

The complete guidelines are available for download in English from the TIF website. The publication was officially launched on 28 January 2025 at the 50th Arab Health Medical Expo, Dubai World Trade Centre.
